We understand that it can be a much-anticipated time when you or a loved one is discharged from hospital. You may be worried about who will look after you or your loved one, you may need some shopping done or support with preparing meals. We believe that you or your loved one’s care should not end the minute they leave hospital. Our team will provide support.

Our team of home carers are at hand to support you with the transition from a hospital to the comfort of your home. Upon your discharge you will get a personalised discharge plan before leaving hospital. This will be drawn up by a team of healthcare professionals and the plans will take into consideration your physical condition, care needs and living arrangements to support you as you settle back into your life and daily routine.

Our experienced clinical manager will meet you or your loved one to discuss the discharge arrangements with you or a family member to arrange a suitable carer for your specific needs. They will carry out an assessment to find out your support needs when you return to your home. In our assessments, we will explore areas such as how you will manage the following daily activities.

  • Will you need support to prepare your meals or will you require medication support?
  • Managing mobility, in or out of the house or any equipment you may require.
  • What support you will need with housework or running errands such as shopping?
  • The level of support you will require with personal care, such as washing or bathing.

Once we have discussed with you or your loved one the care which you are happy with, then we will put together a flexible individualised care plan which suits you or your loved one. Your care plan will be reviewed daily and if you require any aspect of it to change, we will adapt to your needs as they change. Our friendly carers will always be happy to offer a helping hand in the comfort of your home to ensure you have peace of mind.
